Hey there! If you've been wondering about the Goods and Services Tax (GST) on healthcare services in India, you're in the right place.

Whether you're a Chartered Accountant (CA), a business owner, or just someone looking to understand how taxes affect healthcare, this blog is here to make things crystal clear for you.

Let’s dive right into it!

What is GST?

First things first, GST stands for Goods and Services Tax. It’s a comprehensive, destination-based tax that is levied on the supply of goods and services across India.

The tax is designed to simplify the indirect tax system by replacing multiple state and central taxes with a single, unified system.

But, how does this affect healthcare services? Let’s find out.

Are Health Care Services Exempt from GST?

You might be happy to know that most healthcare services in India are exempt from GST. Yes, you heard that right! Medical services like diagnosis, treatment, or care provided by doctors, hospitals, and clinics are not subject to GST.

But hold on, it's not all smooth sailing. There are some gray areas where GST applies.

So, Which Healthcare Services Are Taxed?

Here's where things get a little tricky. While healthcare services are largely exempt, there are some services and products within the healthcare industry that are taxed under GST.

Health-Related Products

Products like medical equipment, implants, and medicines are taxed at varying rates. For example:

  • Medicines: 5% GST
  • Medical devices and implants: 12% GST
  • Bandages, dressings, and similar items: 12% GST

These rates can make medical treatments slightly more expensive as hospitals and clinics need to pass on this cost to patients.

Cosmetic and Plastic Surgeries

Surprisingly, cosmetic surgeries (that are not life-saving) attract GST. Surgeries like liposuction, botox treatments, and rhinoplasty are subject to 18% GST.

However, if the surgery is for reconstructive purposes or medically necessary (say after an accident), it may be exempt.

Health Insurance

If you have health insurance, the premium you pay is taxed under GST at 18%. So, while your medical bills might be exempt, your insurance premium is not.

Ambulance Services

The good news here—ambulance services are also exempt from GST. This ensures that urgent medical transport remains affordable.

What’s the GST Rate on Room Rent in Hospitals?

If you’ve ever been hospitalized, you might know that the room rent can sometimes make up a big part of your medical bill. Here’s where things get interesting.

  • For hospital rooms with a daily charge of less than ₹5000 per day, there’s no GST.
  • However, for rooms costing more than ₹5000 per day, a GST rate of 5% applies.

So, the next time you choose a hospital room, keep the GST impact in mind!

Exemption on Services by Doctors

Medical services provided by doctors, including consultation and surgery, are exempt from GST. This is true whether you’re seeing a general practitioner, a specialist, or even getting treated at a hospital.

The idea behind this exemption is to ensure that basic medical care remains affordable for the average Indian citizen.

GST on Preventive Health Care Packages

Preventive healthcare services, such as health check-ups, vaccinations, and diagnostic tests, also fall under the exempt category. These services are important in keeping us healthy and reducing the risk of serious illness, so it’s great that they remain GST-free.

However, if these services are part of a larger package that includes non-exempt services, GST may be applicable on the non-exempt portion.

What About GST on Medical Tourism?

India has become a popular destination for medical tourism, attracting people from around the world for affordable yet high-quality healthcare.

  • Regarding GST, foreign patients are treated the same as Indian patients. If the service provided is GST-exempt, it’s GST-free for everyone, including international visitors.
  • However, GST does apply on services like luxury hospital rooms, cosmetic surgeries, and any other taxable service that we discussed earlier.

GST Compliance for Hospitals and Healthcare Providers

If you’re a healthcare provider or run a hospital, you need to be mindful of GST compliance. Since most healthcare services are exempt, you won’t be charging GST on these services. But for those services that are taxed (like renting rooms over ₹5000 or selling medicines), you will need to collect and pay GST.

ITC (Input Tax Credit) for Healthcare Providers

One important thing to note is that hospitals and clinics providing exempt services cannot claim an Input Tax Credit (ITC) on their expenses. This can affect the cost structure for healthcare providers, as they must absorb the GST on the goods and services they purchase (like medical equipment, drugs, etc.) without being able to offset it.

How GST Impacts Patients and Healthcare Costs

As a patient, the good news is that your basic healthcare needs—like doctor visits, surgeries, or diagnostic tests—are mostly GST-free. But there are areas where GST comes into play, like your hospital room charges or the medicines and devices used in your treatment.

Because hospitals can’t claim ITC, they might pass on some of the GST costs (on medicines and equipment) to you, making healthcare slightly more expensive in some cases.

What Should You Keep in Mind?

Navigating GST in healthcare can be a bit complex, but here’s the gist:

  • Most healthcare services like doctor consultations, surgeries, and diagnostics are GST-exempt.
  • Be mindful of GST when choosing high-end hospital rooms, cosmetic surgeries, or when paying health insurance premiums.
  • Hospitals and clinics have to follow strict compliance with GST regulations, especially for taxed services and products.

Understanding these basics can help you make informed decisions, whether you’re managing a healthcare business or just trying to keep your medical bills in check.
